Live4Life Ballarat launched on 23 March 2023 and is delivered by engaging the whole Ballarat community; secondary schools, health provider networks and local community organisations.

The lead agency is the City of Ballarat.

Live4Life Central Goldfields is launching in 2022 and will be delivered in partnership with the two secondary schools, Highview College and Maryborough Education Centre and all members of the Central Goldfields Youth Alliance. The lead agency is Central Goldfields Shire Council

Live4Life Bass Coast South Gippsland launched in 2022 and is delivered by engaging the whole community; secondary schools, health provider networks and local community organisations. The lead agencies are South Gippsland Shire and Bass Coast Shire.

Live4Life Southern Grampians was launched in 2020. The model is delivered in partnership with the five secondary schools, Hamilton Parklands, South West Institute of TAFE and eight community agencies; the lead agency is Western District Health Services (WDHS).  

Live4Life was launched in Moira in January 2020. Uniting Vic/Tas are the Lead Agency for Live4Life Moira in partnership with Moira Shire Council, eight local secondary schools, Berry Street and the Salvation Army.

Macedon Ranges Live4Life model was developed in 2009. Since 2010, Macedon Ranges Live4Life is delivered yearly in partnership with five Macedon Ranges secondary schools and four local community organisations; the lead agency is the Macedon Ranges Shire Council.

Live4Life Glenelg was launched in 2017. The model is delivered in partnership with the four secondary schools, South West Institute of TAFE and five community agencies; the lead agency is Glenelg Shire Council.

Live4Life Benalla was launched in 2017. The model is delivered in partnership with the two Benalla secondary schools and the Flexible Learning Centre and eight local community organsiations; the lead agent in Benalla Rural City Council.

Live4Life Baw Baw was launched in 2020 and is delivered in partnership with all secondary schools and other education providers in the Shire as well as six community agencies. The lead agent is Latrobe Regional Hospital.
