Consent Form Audio Visual and Images
Photos and audio-visual content are great ways to tell the story of Live4Life.
Live4Life takes the opinions and rights of young people seriously and we are committed to ensuring that they are active and willing participants in this work.
The Victorian Government’s Child Safe Standards (2021) also requires that young people are informed about their rights, participate in decisions affecting them and taken seriously. It is vital that young people feel empowered and know that it is their choice if they appear in photo or AV content related to Live4Life and that it is a legal requirement young people and their families provide informed Photo and audiovisual consent.
I consent to:
(1) Youth Live4Life Ltd ACN 638 122 648 and its representatives (including contractors) taking photos and/or AV content of me and/or my child/children for the purposes set out in this consent form,
and (2) the use by Youth Live4Life of such photos and/or AV content in publications, promotions, on the Youth L4L website or in other material as set out below, for a period of three years from the date of signing this consent form (Term).
By giving my consent, I acknowledge and agree that the photos and/or AV content featuring me/my child may be used during the Term in the following ways:
in major print publications (eg Youth Live4Life’s annual report, evaluations or strategic plans);
in Youth Live4Life brochures, flyers, newsletters or posters;
on the Youth Live4Life website, social media accounts or in multimedia presentations;
in the media, including media releases to promote Youth Live4Life’s work;
in video productions;
in Youth Live4Life advertisements (eg newspaper, television, website, social media);
for Youth Live4Life retention, storing and archival purposes;
by other Youth Live4Life communities; and/or
by Youth Live4Life for the purposes of complying with its obligations under any applicable laws.
Youth Live4Life and its representatives agree to comply with Victoria’s Child Safe Standards and all other relevant child safety requirements.
Youth Live4Life may publish your first name in photos and/or AV content, but will not publish your full name or any other personally identifiable information in Youth L4L’s publications. If you prefer that your first name not be used in any publications, please inform a Youth Live4Life representative or email us at admin@live4life.org.au
Upon the expiry of the Term, Youth Live4Life agrees to archive any photos and/or AV content relating to you and cease using such content for the creation of new material. However, this does not apply to content published in Youth Live4Life’s publicly available annual reports, published content that is required by law to be retained by Youth Live4Life, or published content that is beyond the control of Youth Live4Life, including, but not limited to, content posted on social media.
Youth Live4Life reserves the right to appropriately crop, change and/or enhance photos and/or AV content taken, without seeking the permission of the subject or their parent/guardian in relation to that editing process.

At any time, young people and or their families, can choose to withdraw this consent. To do this, please inform Live4Life by emailing admin@live4life.org.au, noting that consent may not be able to be withdrawn if the photos/audio visual content have already been published.
Protection of privacy: Youth Live4Life is collecting your personal information in this form in order to seek your consent for the photographs and/or audio-visual footage to be used as set out in this form. Any photographs and/or audio-visual footage of your child taken by Youth L4L may also constitute a collection of your child’s personal information if your child’s identity is clear or can be reasonably worked out from the relevant photograph or audio-visual footage. It may also constitute ‘sensitive information’ if the photograph or audio-visual footage, for example, shows your child’s racial or ethnic origin or religious beliefs. Your personal information and your child’s personal information will be handled in accordance with the Privacy Act 1988 as set out in our privacy policy which is accessible here:
